On Wednesday, December 18th, at approximately 6:16 P.M. troopers with the
Van Wert Post of the Ohio State Highway Patrol were dispatched to the intersection of U.S.
30 at State Route 49 north for a report of a crash involving two commercial vehicles.
The preliminary investigation revealed that Amie Marie Cook, age 31, of New Albany,

lane of westbound U.S. 30, struck Cook’s trailer.
Cook was treated on scene and released with no reported injuries. Blane Williamson
sustained fatal injuries as a result of the crash and was pronounced deceased at the scene
